Overview:
Carle Health is seeking an Adult Psychiatric NP to join our expanding Behavioral Health team at Trillium Place on Jefferson in Peoria, Illinois.
Responsibilities:
Provides health care and demonstrates the knowledge and skills necessary for providing care as an Advanced Practice Nurse to patients in a primary/specialty care setting.
- Demonstrates ability to function both independently and in collaboration with or under the supervision of a physician or group of physicians according to specialty area
- Obtains complete histories and performs complete physical exams on new and returning patients and identifies deviations from normal in the physical assessment
- Evaluates and treats patients with acute and chronic conditions related to the specific specialty area
- Formulates a diagnosis and treatment plan based on the assessment
- Performs, requests, and /or analyzes diagnostic studies as indicated upon evaluation of the patient and assures follow-up of test results if necessary
- Collaborates with physicians as appropriate.
- Initiates appropriate actions to facilitate the implementation of therapeutic plans that are consistent with the continuing healthcare needs of the patient
- Completes written or verbal documentation of patient history, admissions, care plans, progress, discharge notes, etc. in the patient's medical record
- Documents and reports care in accurate, timely manner using appropriate forms and records
- Demonstrates ability to assess and anticipate patient needs.
- Handles phone/electronic messages and pages in a timely and appropriate manner.
- Accurately records telephone conversations in the medical records.
- Maintains certification by attending the appropriate seminars and conferences.
- Continues to keep informed of current standards of practice for nurse practitioners.
- Possesses knowledge and skills necessary to obtain adequate lab specimens for testing.
- Performs in-office patient lab testing and quality control in compliance with medical provider’s orders and office lab procedures/policies.
Coordinates patient care activities and the flow of patient information by effective communication with other office staff, physicians, patients, families and a variety of health care agencies and appropriate documentation to ensure continuity of patient care.

- Provides and discusses treatment options with patients and family members
- Maintains communication with family members
- Assists in the management of family dynamics and coping mechanisms during acute and chronic phases of patient care
- Discusses appropriate information regarding patient care with the collaborating or supervising physician
- May dictate follow-up letters to referring healthcare providers with summaries of treatment, response, and plan according to specialty area as appropriate
- Effectively communicates and interacts with patients and families from diverse backgrounds
- Establishes relationships with professional and/or other health-related groups within the organization and community.
- Shares clinical expertise with professional and supportive personnel
- Collaborates with other health disciplines to ensure continuity and quality of care
- Uses interpersonal skills to work productively with all levels of hospital personnel
- Assists with orientation of new staff when appropriate.
- Conducts appropriate and timely documentation of patients concerns, problems and/or symptoms in medical records.
- Serves as a preceptor to NP students.
- Requests written consultation from physicians or other healthcare professionals to ensure appropriate and quality patient care
- Participates in staff development and in-service programs
Provides patient education through application of teaching-learning principles appropriate to the age and needs of the patient population served by the office. To help patients prevent and/or effectively cope with medical problems and to promote a healthy lifestyle.
- Demonstrates the knowledge and skills of age appropriate care. Assesses information and data indicative of the patient's learning capabilities.
- Interprets information relative to the age-specific needs of the patients.
- Promotes effective communication by providing effective learning techniques to patient and family members.
Promotes/Supports Quality Improvement initiatives within the practice setting as well as within the Medical Group and the Medical Center
- Participates in the planning/implementing actions to address and improve Patient/Employee satisfaction
- Supports the regulatory process by assisting with the inspection preparation and maintenance of standards of care
- Maintains provision of preventive health services (e.g. screening, risk assessment, immunizations, PAP smears, mammograms, colonoscopy, etc.)
- Applies current concepts and evidence-based findings from research and/or studies to practice
- Initiates change in clinical practice based on current evidence-based concepts and findings
- Works to improve patient outcomes through various initiatives, committees and work groups
- Enhances the ability of others to provide health care by acting as a role model
- Maintains professional licensure
- Continues professional development through participation in continuing education
- Supports the Access and Efficiency program through participation in meetings, measures and/or process change initiatives
- Provides opportunities for staff development
- Promotes the use of and/or develops patient education programs/materials
- Participates in Medical Group activities, subcommittees, task teams, work groups as requested
