Brief Job Description:
This position performs highly advanced social services and case management work for Children and Pregnant Women programs, Newborn Screening, Childhood Lead Poisoning Prevention Program, Personal Care Services, Children’s Health Insurance Program, Child Protective Services, and Children with Special Health Care Services Program in Public Health Region 9/10.
Duties include conducting comprehensive assessments to determine eligibility and need, travelling to conduct home visits, and completing intakes, Family Needs Assessments, Service Plans, and Follow-up Services to families on caseload; Links families to community resources and provides information and referral services; Conducts presentations and communicates program services and changes to consumer/public in a positive and accurate manner. Actively participates and advocates in Community Resource Coordination Groups, case management coalitions, and community organizations; Completes administrative duties, special assignments, and monthly reports.
Essential Job Functions (EJFs):
EJF 1: Maintains case files, provides specialized social work consultation and assessment to families of children who are medically fragile and other populations to determine personal care services eligibility, and enters family encounter information into database. Conducts needs assessments, completes Individual Service Plans, and follows up to determine if services have been rendered. May attend school meetings or group conferences for clients. (45%)
EJF 2: Collaborates with community groups, public health entities, and internal staff and others to promote efforts to service families of children with health risks, conditions, and special health care needs. Provides presentations and information to stakeholders and attends trainings and seminars to gain knowledge of community resources. (20%)
EJF 3: Reports activities to meet regional program goals and objectives and ensures case related and community activities are documented in database. (15%)
EJF 4: Creates corrective action plans for quality assurance file based on audit results and maintains documentation on TWICES. Assists with training new staff, volunteers, and social work interns. Participates in peer review for quality assurance. (15%)
EJF 5: Other duties as assigned including programmatic processes for events and conferences, maintaining knowledge of applicable policies, and actively participating in a supporting role to meet the agency’s obligation for disaster response and Continuity of Operations. (5%)
Knowledge, Skills and Abilities (KSAs):
Knowledge of case management principles and practices, children’s programs, quality assurance and technical assistance policies and procedures, community resources, and cultural and socioeconomic issues related to health and disabilities.
Skill in compiling reports, conducting in-home family needs assessments, developing service plans, presenting, training, conflict resolution, mediation, and using computer software programs (Microsoft Word, Excel, Access, Outlook, PowerPoint).
Ability to interpret and apply policy and procedures, provide professional leadership, maintain confidentiality, and provide own transportation and maintain acceptable driving record.
Registrations, Licensure Requirements or Certifications:
1. Active Social Work license or current license to practice as a Registered Nurse in the State of Texas.
2. Valid Class C Texas driver’s license, or equivalent from another state.

2. Experience in case management principles and practices is required. Two years’ experience is preferred.
3. Experience with local resources is required. Two years’ experience is preferred.
4. Experience using effective organizational skills to meet deadlines is required. Two years’ experience is preferred.
5. Experience using computers and related equipment is required (Microsoft Office Suite, databases, and presentation software). Two years’ experience is preferred.
